Convergence Integrated Care
Integrating care. Improving outcomes. Serving Minnesota.
What is CIC?
Convergence Integrated Care (CIC) is a statewide clinically integrated network of 14 independent non-profit community behavioral health agencies across Minnesota. Member agencies formed this network to create a system of community behavioral health care.
CIC provides high-quality care and a consistent high-quality experience of care across its full network population — individual clients, families, communities — through coordinated care pathways, improving access to services and treatment and closing referral loops for clients. CIC brings a local network of providers with expertise, knowledge, and relationships with local and state communities.
Why CIC?
MACMHP created Convergence Integrated Care to drive innovation in service delivery and advance payment reform — making programs and models more equitable and consistent across providers. If MACMHP’s vision is a healthcare system where everyone has access to high-quality, community-based services, CIC is one of the most important vehicles for getting there.
CIC’s Impact
Value-Based Care
CIC is built on a belief that better integration of care leads to better outcomes — for patients, for providers, and for the system as a whole.
What CIC Does
CIC’s staff team provides member agencies with services to help them build integrated service models, improve quality and outcomes for their clients. The network assists member agencies in designing and improving their work in value-based care and new innovations in service delivery.
CIC Goals
CIC’s goals are to advocate for and build an integrated system that prioritizes and meets clients’ needs where they are. CIC is also working to give an identity to the work being done — helping redefine community behavioral health care.
Standardizing Care
CIC is building network-level standards of care across community behavioral health agencies, decreasing variation in service quality and client experience across network providers. CIC accomplishes this by building data interpretation and decision-making capacity — collecting, analyzing, and reporting population health data — to support providers’ delivery of care and treatment for clients.

CCBHC — Certified Community Behavioral Health Clinics
Page Headline
A new model for behavioral health care in Minnesota.
What is CCBHC?
Certified Community Behavioral Health Clinics (CCBHCs) are a federally recognized model of care designed to expand access to comprehensive behavioral health services. CCBHCs are required to provide a full range of mental health and substance use disorder services, regardless of a patient’s ability to pay.
The Minnesota Model
Minnesota has been a national leader in CCBHC implementation. The Minnesota Model represents the state’s approach to certification — building on the strength of existing community-based providers to create a more integrated, equitable system of care.
Why It Matters
MACMHP created the CCBHC initiative alongside CIC to improve service delivery and make payment models more consistent across providers — serving some of Minnesota’s most vulnerable patients, including individuals experiencing serious mental illness, substance use disorders, and co-occurring conditions.
