Convergence Integrated Care (CIC) is a statewide clinically integrated network of 12 independent non-profit community-based mental health agencies across Minnesota. 

Vision

At CIC, our vision is to move beyond standard care delivery and establish our clinically integrated network as a Center of Excellence (COE) for community mental health that prioritizes and meets our clients’ needs where they are. We are building on what we do best: working together, using data creatively, and delivering care to those who need it most. By operationalizing the unique expertise of our members, we create a multiplier effect that drives systemic change. We are breaking down silos to build a unified and cohesive ecosystem that maximizes our impact, moves beyond fragmented care to a model of high-performance delivery that stabilizes the network’s future and the lives of those we serve.

By uniting a local network of providers with deep expertise and trusted community relationships, we bridge gaps and build stronger pathways to wellness.

Created by the Minnesota Association of Community Mental Health Programs (MACMHP), Convergence Integrated Care (CIC) is the driving force turning a bold vision into reality: a clinically integrated system where clients have seamless access to high-quality, community-based services. 

As healthcare becomes increasingly complex, CIC serves as the vital vehicle for change, driving innovation in care delivery, informing and advancing equitable payment reform, and bringing equity, consistency, and clinical excellence to the communities in our network.
