Convergence Integrated Care

Integrating care. Improving outcomes. Serving Minnesota.

What is CIC?

Convergence Integrated Care (CIC) is a statewide clinically integrated network of 14 independent non-profit community behavioral health agencies across Minnesota. Member agencies formed this network to create a system of community behavioral health care.

CIC provides high-quality care and a consistent high-quality experience of care across its full network population — individual clients, families, communities — through coordinated care pathways, improving access to services and treatment and closing referral loops for clients. CIC brings a local network of providers with expertise, knowledge, and relationships with local and state communities.

Why CIC?

MACMHP created Convergence Integrated Care to drive innovation in service delivery and advance payment reform — making programs and models more equitable and consistent across providers. If MACMHP’s vision is a healthcare system where everyone has access to high-quality, community-based services, CIC is one of the most important vehicles for getting there.

CCBHC — Certified Community Behavioral Health Clinics

Page Headline

A new model for behavioral health care in Minnesota.

What is CCBHC?

Certified Community Behavioral Health Clinics (CCBHCs) are a federally recognized model of care designed to expand access to comprehensive behavioral health services. CCBHCs are required to provide a full range of mental health and substance use disorder services, regardless of a patient’s ability to pay.

The Minnesota Model

Minnesota has been a national leader in CCBHC implementation. The Minnesota Model represents the state’s approach to certification — building on the strength of existing community-based providers to create a more integrated, equitable system of care.

Why It Matters

MACMHP created the CCBHC initiative alongside CIC to improve service delivery and make payment models more consistent across providers — serving some of Minnesota’s most vulnerable patients, including individuals experiencing serious mental illness, substance use disorders, and co-occurring conditions.
